Root Cause Analysis and 8D: fix it once, properly.

Most problems come back because the team treated a symptom and called it a cure. This bootcamp gives you four root cause approaches and over forty tools, so you can find what is actually causing the failure and put in a corrective action that holds.

The path

Five stages between a problem and a permanent fix

Skip any one of them and the problem comes back. This is the spine that runs through all four approaches taught in the course.

1
Define

Separate the problem from the symptom and state it precisely, using 5W2H, SIPOC and a Gemba walk

2
Contain

Protect the customer now with interim containment, identification and traceability

3
Analyze

Find the true cause with Five Whys, cause and effect analysis, data tools and structured team techniques

4
Correct

Develop and implement permanent corrective actions, then track them with Gantt charts and a RACI matrix

5
Prevent

Stop recurrence for good with poka-yoke, control plans and FMEA, then recognize the team

Four approaches

One method never fits every problem

A jammed printer and a field failure costing six figures do not deserve the same process. Learn all four and pick the one the problem warrants.

1️⃣

Five Whys

The fastest route from symptom to cause. Theory, a working template, and a demonstration of how to keep asking why without drifting into guesswork.

2️⃣

Cause and Effect Analysis

The fishbone, or Ishikawa, diagram for problems with many candidate causes. Built three ways: in Excel, in SigmaXL and in Minitab Workspace.

3️⃣

A3 Problem Solving

The Lean one page discipline, with is and is not analysis, the seven basic quality tools, descriptive statistics, charting and value stream mapping behind it.

4️⃣

8D Problem Solving

The full team based method used in automotive and aerospace, from D0 emergency response through to D8 recognition, with a complete 8D template.

What you'll learn

Skills you can take into the next problem review

Tell a problem apart from its symptoms, and state the problem in a way a team can actually work on
Choose the right root cause approach for the situation instead of forcing every problem through one method
Distinguish correction, corrective action and preventive action, and know which one the situation calls for
Run the full 8D discipline, from emergency response and interim containment through to preventing recurrence
Build and lead a problem solving team, recognizing Tuckman's stages, conflict patterns and negative dynamics
Apply the seven basic quality tools and read what the data is telling you rather than what you expected
Use descriptive statistics and the right chart for the job, from box plots and histograms to time series
Generate and organize candidate causes with brainstorming, nominal group technique and affinity diagrams
Design corrective actions that hold, using poka-yoke, force field analysis, control plans and benefit cost analysis
Prevent recurrence with FMEA, and map a value stream to see where the waste and the delay actually sit
Course curriculum

Who it's for

Anyone who gets handed the problem nobody has solved yet

Some basic familiarity with quality management concepts is all you need to start.

Quality managers and engineers

Customer complaints, internal failures and audit nonconformities all land with you. This gives you a defensible method for every one of them.

Continuous improvement teams

You already run projects. This adds the analysis depth that separates a real root cause from the first plausible explanation.

Process analysts

The statistics, charting and value stream mapping sections give you the toolkit to show what the data actually supports.

Anyone who owes a customer an 8D

Automotive and aerospace customers ask for 8D reports by name. Learn the discipline properly rather than filling in the form afterwards.

Supervisors and team leads

The team sections on roles, Tuckman's stages, conflict and negative dynamics are as useful in a daily meeting as in a problem solving team.

Six Sigma practitioners

Green and black belts will recognize much of the toolkit, and will find the A3 and 8D disciplines a fast way to structure smaller problems.

Sandeep is an engineer and quality management expert with more than 40 years of experience leading problem solving teams and coaching others to do the same. The tools in this course are the ones he has used on real failures, which is why every one of them comes with a demonstration rather than a definition.

His approach to teaching root cause analysis: show the method, show the tool, then show it being used, so that what you learn survives contact with a messy real problem.

FAQ

Common questions

What background do I need before starting?
Some basic familiarity with quality management concepts is helpful, for example knowing what a nonconformity or a corrective action is. You do not need a Six Sigma belt, a statistics background, or prior experience running a problem solving team. Every tool is introduced from first principles and then demonstrated.
Why four approaches rather than one?
Because problems differ in size and stakes. A recurring minor irritation is well served by Five Whys in twenty minutes. A customer escalation with a containment obligation needs the full 8D discipline and a cross functional team. Learning all four means you match the method to the problem instead of over engineering small issues and under investigating big ones.
Do I need Minitab or SigmaXL to take this course?
No. The demonstrations show the tools being built in Excel as well as in SigmaXL and Minitab Workspace, so you can follow along with whatever you have. The methods themselves are software independent, and the reasoning behind each tool is taught before any software appears.
Is this the same as your Six Sigma courses?
No. Six Sigma courses teach the DMAIC project framework end to end. This bootcamp focuses specifically on finding root causes and making corrective actions stick, using four approaches that work on their own without a full project structure. There is overlap in the toolkit, so Six Sigma practitioners will recognize a lot, and many still find the A3 and 8D disciplines a faster route for smaller problems.
What is the difference between correction, corrective action and preventive action?
A correction fixes the item in front of you, such as reworking a defective part. A corrective action eliminates the cause so the same problem does not recur. A preventive action removes the cause of a problem that has not happened yet. The course covers all three early, because confusing them is the single most common reason corrective action plans get rejected.
What are the PMI PDUs and SHRM PDCs for?
PDUs (Professional Development Units) count toward maintaining your PMI certification, such as the PMP or CAPM. PDCs (Professional Development Credits) count toward maintaining your SHRM certification, SHRM-CP or SHRM-SCP. Both are awarded on successful completion of the course. Quality Gurus Inc. is a PMI Authorized Training Partner (ATP #6034) and a SHRM Recertification Partner.
What is the CPD Group (UK) accreditation?
The CPD Group is one of the UK's most recognised accreditation bodies for continuing professional development. Their accreditation confirms that this course meets their standards for quality and educational value, under Provider #784310, Accreditation #1016191. The accreditation is accepted by professional bodies, employers and licensing authorities worldwide.
How long do I have access?
